you need to provide the blog ownership by connecting your blog to your Google+ account ,Google Analytic and Webmaster tool accounts.
Ads Networks
Do not use any other ads network ads on your blog .You request will not be approved if you have any other ads publisher ads on your blog .Remove all of them if you have on your blog or website.
